I am pleased that the editorial titled "Does Sex Matter in Glaucoma?"¹ met its objective of stimulating discussion regarding the potential relationship between glaucoma and sex, as evidenced by the thoughtful letters that were submitted. Dr Mark is correct in pointing out the greater prevalence of angle-closure glaucoma among women compared with men. Considering that the prevalence rates for hyperopia are higher among women, particularly those older than 60 years,² there is an anatomic predisposition to angle-closure glaucoma. Recently, investigators from southern Croatia noted that the relative risk for developing angle-closure glaucoma is 2.1 times higher for women than for men.³

Ms Podos’ question about the length of exposure to estrogen during a woman’s lifetime as a contributor to the early onset of glaucoma is more difficult to answer.
